When the cushion between the vertebrae comes under pressure
Disc-related issues refer to changes in the cartilaginous cushions between the vertebral bodies – ranging from age-related loss of disc height to bulging (protrusion) and disc herniation, where tissue escapes through the disc's outer fibrous casing. Not every change visible on imaging is accompanied by symptoms: many people without back pain show similar findings.
Back pain is one of the most common complaints in dance: a systematic review found a yearly prevalence of around 73 percent among dancers. Movements such as the deep backbend (cambré), a high arabesque, or repeated combinations of trunk flexion and rotation in contemporary dance place particular strain on the lumbar spine and its discs. In adolescents, a rate of stress reactions in the spine (medically known as spondylolysis) up to four times higher than in the general population has also been described, often linked to forced hyperextension combined with insufficient trunk control. Interestingly, imaging studies in adult dancers have not shown greater disc degeneration compared with non-dancers – suggesting that technique, core stability, and individual movement quality play a larger role than dance loading itself. Back pain in dance therefore rarely results from a single event, but more often from an interplay of technique, training volume, and muscular control.
Given the high prevalence of back complaints in dance, early attention to core control, clean technique, and adapted load management is an important part of CENIT e.V.'s prevention work. Understanding how hyperextension can be supported safely, rather than forced, actively helps to avoid long-term back problems.
